Scenario: All going well until he/she strips off - OMG like haven't showered for days or can smell the ass ... ... fortunately doesn't happen often but recently came up against this situation.

It's only happened to me once that goodness. I suggested a shower so we could both "start out fresh". She liked the idea and it turned out to be a good night!
It happened to me with a very beautiful, built like a brick house woman. She didn't use deodorant. It was tolerable while making out, but I couldn't get turned on by anyone similar to her for months.
I do know one person that just says "Listen, I'm sorry, I'm sensitive to smells, could you take a shower?" and sees if it fixes it.

I think the "let's take a shower together" approach is safest and least likely to offend.
Thankfully never been in that position. I do hope that if I ever was, I would have enough self respect to leave immediately. Not sure why you want to be sensitive to a person that isn't sensitive to you. Imagine if they don't use a bar of soap, there wouldn't be much hope that they use a toothbrush.
